Appointment Policy
We know that your time is valuable and we want to be able to accommodate scheduled appointment times.
Any arrivals more than 10 minutes past your appointment time will need to be rescheduled.
If you are unable to keep your appointment, please contact our office or reach out on the patient portal 24 hours in advance of your scheduled appointment time. Failure to do so will result in a $25.00 fee.
New Patients
We ask new patients to arrive 10 minute early. Please bring the following items with you to your appointment.
- Photo ID
- Insurance Card
- A Form of Payment
- All Prescription Medication Bottles
Follow-up Appointments
For follow-up appointments, please bring the following items with you.

Prescription Refill Policy
To request a prescription refill, please contact your pharmacy or make a request via our secure patient portal.
Billing/Finance Policy
All co-pays and balances are due at the time of your appointment.
